Your First Visit

On your first visit, be sure to arrive 15 minutes early to complete the registration process. Bring a list of all medications, including dosage, you are currently taking, as well as any recent x-ray films or lab work pertaining to the reason you are being seen. Also, bring your insurance card(s) so we may bill your insurance company directly when appropriate.

The following forms are provided as a convenience. You can save time by printing them out and bring them completed to your first appointment.

Appointments

Our office is open Monday through Friday, 9:00 a.m. to 5:00 p.m. Office hours are by appointment only.

Telephone Calls

Our trained staff will do their best to answer all of your questions. Please call 978.372.7751 (Haverhill) or 978.462.7751 (Newburyport) during office hours. After hours the telephone system will direct you to leave a message. For an emergency you will be directed to call our answering service who will contact the physician on-call. If your call is about billing or insurance question, please call 978.372.5634.

Confidentiality

Prescription Refills

Prescription refill requests need to be called into the office Monday through Friday between 9:00 a.m. to 3:00 p.m. There will be no refill if you have not had an appointment within a year.

Payment

Payment is due at time of service, unless other arrangements have been made. We accept cash, checks, Visa, MasterCard, and Discover. Please feel free to discuss any financial concerns with our billing department. We are willing to work with you to establish a reasonable payment plan. All co-payments are due at time of service.
